Scan Ijazah dan Transkrip Sarjana: IPK minimal 2,90 di bidang Ilmu Farmasi.Akreditasi: Ijazah Sarjana Farmasi sebelumnya harus memiliki akreditasi minimal "Baik".Surat Pernyataan Kemampuan Biaya: Harus diunduh dari situs web pendaftaran.Surat Keterangan Kesiapan Penempatan PKPA.Semua dokumen yang tercantum di atas harus diunggah di halaman pendaftaran PMB (tersedia di s.ung.ac.id/Apoteker2).Persyaratan ini wajib dipenuhi untuk proses pendaftaran, guna memastikan calon mahasiswa memenuhi standar akademik dan profesional yang ditetapkan oleh program ini.
